Abstract
The adrenal glands are pyramidal structures which lie adjacent to the upper poles of the kidneys. The main types of steroids produced by the adrenal cortex are mineralocorticoids (principally aldosterone), glucocorticoids (principally cortisol), and androgens. Steroidogenesis in the zona fasciculata (primarily cortisol) and reticularis (mainly androgen) is under hypothalamic–pituitary control; secretion of aldosterone from the zona glomerulosa is under the control of the renin–angiotensin system. This chapter provides information on investigations of adrenocortical function, glucocorticoid excess, glucocorticoid deficiency, mineralocorticoid excess, mineralocorticoid deficiency, sex steroid excess, adrenal enzyme defects, sex steroid deficiency, and adrenal medullary disorder. It also includes information on when to involve a specialist centre, case histories, and useful information for patients and parents. It is particularly important to consider adrenal insufficiency in a newborn infant with unexpected hypoglycaemia, for example, when neither the gestation nor birth weight explains this problem, and especially if there is concurrent jaundice.
