Dziecięcej, Uniwersytet Medyczny w LublinieZespół Stevensa-Johnsona (SJS) i toksyczna nekroliza naskórka (TEN) wraz z zespołem nakładania SJS/ TEN tworzą spektrum ciężkich, zagrażających życiu reakcji polekowych.Postępowanie terapeutyczne w przypadku tych stanów chorobowych nadal pozostaje nieustalone, ponieważ różne opisywane metody leczenia nie są oparte na dużych kontrolowanych badaniach, ale na doświadczeniach poszczególnych ośrodków, opisach pojedynczych przypadków lub niewielkich serii przypadków.Zgodność poglądów dotyczy jedynie konieczności natychmiastowego zaprzestania stosowania leku wywołującego i podstawowego znaczenia postępowania podtrzymującego funkcje życiowe.Wśród najczęściej stosowanych terapii wymienia się glikokortykosteroidy, dożylne immunoglobuliny, cyklosporynę, cyklofosfamid, plazmaferezę i inhibitory TNF-α, jednak wszystkie znane metody mają ograniczenia i wiążą się z ryzykiem wystąpienia potencjalnych efektów niepożądanych.Rola systemowych glikokortykosteroidów w leczeniu SJS i TEN jest nadal kontrowersyjna.Niektóre wcześniejsze publikacje wiązały steroidoterapię ze zwiększoną śmiertelnością, wyższą częstością występowania posocznicy, przedłużoną hospitalizacją, wydłużonym czasem gojenia, jednak dalsze doniesienia nie potwierdziły tego związku.Analiza danych pochodzących z wielu ośrodków wskazuje, że wczesna terapia pulsowa glikokortykosteroidami pozwala uzyskać korzystną odpowiedź kliniczną i poprawia wskaźniki przeżycia w TEN.
