This is a progress report which reviews reflux gastritis with emphasis on the physiopathology, the diagnosis and the medical and surgical treatment. All the important contributions to this elusive and difficult clinical problem since its initial description are included. The clinical, endoscopic and histologic definition of this syndrome with a specific classification and a precise description of the physiopathological background of alkaline reflux gastritis is given. The importance of a preoperative psychiatric evaluation and the importance of studying the preoperative gastric emptying on the outcome of surgery are stressed. A collective review of the medical treatment and the different surgical operations to prevent the duodenal content reflux into the stomach is presented and analyzed. The importance of the selection of patients for surgery is emphasized.
