Amphotericin B Disposition After Aerosol Inhalation in Lung Transplant Recipients

AbstractBACKGROUND:Bronchopulmonary fungal infections continue to be a major cause of morbidity and mortality in lung transplant recipients, and amphotericin B remains the drug of choice for prophylaxis of most fungal infections. Unfortunately, intravenous amphotericin B has numerous serious adverse effects; thus, nebulized amphotericin B could decrease the incidence of adverse effects seen with the intravenous formulation and provide high local concentrations in the lung tissue. We performed a prospective pilot study to characterize the bronchoalveolar lavage (BAL), lung tissue, and plasma concentrations of amphotericin B following inhalation administration to lung transplant recipients.METHODS:Amphotericin B 30 mg was administered by nebulizer prior to a routine bronchoscopy. Amphotericin B concentrations in BAL samples from the upper and lower lobes, transbronchial biopsies, and plasma (obtained by drawing a blood sample 30 min after the amphotericin B inhalation) were analyzed by HPLC.RESULTS:Eight patients were enrolled in the study (mean age 50.0 ± 16.1 y; number of years posttransplant 3.0 ± 1.9; type of transplant 5 double-lung, 3 single-lung). The mean amphotericin B concentration in the upper and lower lobe BAL samples were 0.68 ± 0.36 and 0.50 ± 0.31 μg/mL, respectively. Amphotericin B concentrations, detected in only 2 of 5 biopsy samples, were 0.118 and 0.03 μg/g. Amphotericin B was detected in the plasma of only 1 patient (0.19 mg/L).CONCLUSIONS:This pilot study demonstrated that detectable concentrations of amphotericin B can be attained in both the upper and lower BAL samples following aerosol administration. However, the frequency of the dose and duration of treatment still need to be determined in a larger study. ResumenOBJETIVO:Infecciones fúngicas broncopulmonares continuan siendo una mayor causa de morbilidad y mortalidad en pacientes que hayan recibido transplantes de pulmón. Amfotericina B también permance ser el agente de primera elección en el tratamiento profiláctico contra la mayoría de infecciones fúngicas. Desgraciadamente, el uso intravenoso de amfotericina B posee un gran número de efectos secundarios los cuales previenen su uso en el tratamiento de estos pacientes. Reconociendo esta desventaja, se considera que la administración de amfotericina B via nebulización podra disminuir la ocurrencia de los efectos secundarios atribuidos a la administración intravenosa de amfotericina B y, a la misma ves, proveer una concentración alta de este fármaco en el tejido pulmonar. Se ejecutó un estudio piloto con el propósito de caracterizar las concentraciones de amfotericina B en el lavado broncoalveolar, tejido pulmonar, y plasma de pacientes que hayan recido transplantes pulmonares después de su administración via inhalación.MÉTODOS:Se administró 30 mg de amfotericina B por nebulización antes de broncoscopia rutinaria. Concentraciones de amfotericina B fueron medidas por medio cromatografía líquida del alto rendimiento en muestras obtenidas del lavado broncoalveolar en lóbulos superiores e inferiores, de biopsias transbronquial, y de plasma (estas fueron obtenidas de una muestra de sangre 30 minutos después de la inhalación de amfotericina B).RESULTADOS:Ocho pacientes fueron incluidos en este estudio y tenían las siguientes demograficas: edad promedia de 50.0 ± 16.1 años; número de años post transplante 3.0 ± 1.9; y tipo de transplante, 5 pacientes con transplante bipulmonar y 3 con transplante unipulmonar. Las concentraciones promedio de amfotericina B en muestras del lavado broncoalveolar de los lóbulos superiores e inferiores fueron de 0.68 ± 0.36 μg/mL y 0.5 ± 0.31 μg/mL, respectivamente. Concentraciones de amfotericina B fueron detectadas en solamente 2 de las 5 muestras de biopsia y consistieron de 0.118 y 0.03 μg/g. También se detectó amfotericina B en el plasma de un solo paciente (0.19 mg/L).CONCLUSIONES:Se demuestra en este estudio piloto que se puede obtener concentraciones detectables de amfotericina B en muestras del lavado broncoalveolar en lóbulos superiores e inferiores después de la administración del fármaco via nebulización. Sin embargo, la frequencia de las dosis y la duración del tratamiento de amfotericina B deben ser determindas en un estudio mayor. RésuméOBJECTIF:Résumé les infections fongiques broncho-pulmonaires sont une cause majeure de morbidité et de mortalité chez les transplantés pulmonaires et l' amphotéricine B (AB) demeure le médicament de choix pour la plupart de ces infections. Malheureusement, l'AB intraveineuse comporte plusieurs effets secondaires qui peuvent potentiellement être minimisés avec une administration en nébulisation, tout en atteignant des concentrations plus importantes au niveau pulmonaire. Cette étude pilote prospective a pour but d'identifier les concentrations d'AB dans les tissus pulmonaires, le plasma, et dans le liquide de lavage broncho-alvéolaire (LBA) après une administration par inhalation chez des transplantés pulmonaires.MÉTHODE:Trente milligrammes d'AB ont été administrés par nébulisation avant une bronchoscopie de routine. Les concentrations dans le liquide de LBA, dans la biopsie transbronchique, et le plasma (prélevé 30 minutes après l'inhalation) ont été analysés par chromatographie liquide à haute performance (HPLC).RÉSULTATS:Étude faite avec 8 patients (âge moyen 50.0 ± 16.1 ans; moyenne de 3.0 ans post-transplantation, 5 transplantations doubles, 3 transplantations simples). La concentration moyenne d'AB dans les liquids de LBA était respectivement de 0.68 ± 0.36 μg/mL et 0.50 ± 0.31 μg/mL dans les lobes supérieurs et inférieurs. Une concentration d'AB était detectable dans 2 des 5 biopsies (0.118 et 0.03 μg/g) et dans un seul échantillon plasmatique (0.19 mg/L)CONCLUSIONS:Cette étude pilote démontre qu'une administration en nébulisation d'amphotéricine B peut produire des concentrations décelables dans le liquide de lavage broncho-alvéolaire.
