This chapter deals with otitis in dogs and cats, and focuses on otitis externa and otitis media. It discusses the etiology/pathophysiology, signalment, history and clinical signs, diagnosis, and therapy of otitis. In otitis externa, the primary disease initially causes inflammation within the canal which leads to an increase in epidermal turnover and hyperplasia of the canal wall with narrowing of the lumen. Primary disease triggered by atopic dermatitis is most commonly seen in the West Highland white terrier, Staffordshire bull terrier, Labrador retriever, German shepherd dog, and boxer. In most cases, infectious otitis media is secondary, resulting from a descending infection as an extension of otitis externa, rather than an ascending infection from the oropharynx or a result of hematogenous infection. Therapy involves several stages: flushing the bulla; infusion of topical medication(s) into the bulla; reduction of inflammation with glucocorticoids (topical or systemic); administration of systemic antibiotics; and reassessment every 7–10 days.
