P1016 STUDY OF VISCERAL SENSITIVITY MEASURED BY RECTAL BAROSTAT IN PEDIATRIC PATIENTS SUFFERING FROM FUNCTIONAL GASTROINTESTINAL DISORDERS

Abstract
Introduction: A decreased threshold of visceral sensitivity measured by a rectal barostat is used as a marker of irritable bowel syndrome (IBS) in adults. Our purpose is to 1) evaluate the feasibility of rectal barostat testing in pediatric patients, 2) measure the threshold of rectal visceral sensitivity in children with functional gastrointestinal disorders (Rome II criteria), 3) determine if the characterization of the pain described during the barostat study is similar to the pain reported at home. Methods: Nine patients with functional abdominal pain (FAP), four with IBS, two with Crohn’s disease, aged 8 to 17 years, underwent a series of rectal isobaric balloon distensions (from 2 mmHg to a maximum of 32 mmHg) using an electronic barostat. The patients’ perception (gas, stool, discomfort, pain) was noted during each distension. Rectal sensory threshold was defined as the pressure in the balloon when abdominal pain was experienced. Pain was quantified according to the Visual Analogue Scale. Children were also asked to draw on a standardized human body diagram (the degree of the pain being represented by three colors) their pain experienced at home and during the procedure. Results: The barostat study was well tolerated by all patients. In all four IBS patients, the usual pain was reproduced at a median threshold of visceral sensitivity of 12 mmHg (Range 8–16 mmHg). In 7 of 9 patients with FAP, the pain was felt at a median level of 26 mmHg (Range 8–30 mmHg). In the 2 remaining patients with FAP the procedure failed because of technical difficulties. In all patients with IBS or FAP the localization and the intensity of the pain experienced during the procedure correlated with the pain experienced at home. In the two patients with Crohn’s disease (one having recurrent pain despite treatment and the second diagnosed in retrospect) the usual pain was not reproduced at 32 mmHg. Conclusion: The rectal barostat test can be safely used and is well tolerated in children. Those with functional disorders seem to present a lower threshold of rectal sensitivity, and the pain experienced at home is highly reproducible during the barostat study. The study is in progress to evaluate normal values of rectal sensitivity threshold and a larger patient population with different functional gastrointestinal disorders to confirm our preliminary data.
