Unexplained total failure of fertilization after standard IVF occurs in 1.5%-10.5% of cycles, and is more common in patients with unexplained infertility. Re-insemination of oocytes yields very poor results. Rescue-ICSI of unfertilized oocytes 22 hours post retrieval yields 48-60% fertilization rate, with a pregnancy rate of 5-17%. Rescue-ICSI of unfertilized oocytes 6 hours post retrieval yields 70% fertilization rate, with better quality embryos and a pregnancy rate of 48%, but requires change in the standard practice of embryology laboratories.
