Abstract
Cardiogenic shock is a clinical state in which target tissue perfusion is inadequate to supply vital substrates and remove metabolic waste. This end-organ hypoperfusion is usually caused by heart failure (HF). Inadequate cellular oxygenation leads to marked generalized impairment of cellular function and multiorgan failure. Autopsy studies indicate that combined new and old extensive myocardial infarction (MI) consistently involve more than 40% of the left ventricular (LV) myocardium. Hemodynamic parameters include: persistent hypotension (systolic blood pressure <80–90 mmHg or mean arterial pressure 30 mmHg lower than the baseline) with severe reduction in cardiac index (<1.8 L/min/m 2 without support or <2.0–2.2 L/min/m 2 with support) and adequate or elevated filling pressure (e.g., LV end-diastolic pressure >18 mmHg or right ventricular [RV] end-diastolic pressure >10–15 mmHg) (Reynolds and Hochman 2008 ).
