Abstract
With increased access to antiretroviral therapy, people living with HIV (PLWH) are living longer and healthier lives. Despite treatment efficacy, studies show that older PLWH evidence significant burdens of disease as they age, as seen in higher levels of non-HIV related comorbidities, such that multimorbidity is the rule and not the exception. Normative aging-related stressors, such as chronic illness, disability, and social losses can undermine health outcomes. For many, these stressors are exacerbated by poverty, unemployment, stigma and discrimination, social isolation, and housing and food insecurity. Despite these complexities, older PLWH can achieve optimal aging with HIV. This paper explores the shifting notions of successful aging and programmatic applications. We present a new approach for older PLWH at risk that includes viral suppression, HIV self-care management and the reduction of risk behaviors (e.g., smoking, substance abuse) to maintain physical and psychosocial functional ability and foster wellbeing in older age.
