Abstract
Pelvic ring disruption in a young patient is extreme and often life threatening. The priority is saving the patient's life and then managing the pelvic fracture in order to reduce the high morbidity associated with pelvic fractures. During resuscitation, early stabilization for controlling pelvic fracture-related haemorrhage is essential. Evidence suggests that non-invasive stabilization devices like a simple bed sheet are as effective as an anterior external fixation. Priority may then be given to immediate angiography. A significant number of survivors will suffer major chronic disability with long-term problems. Fixation of all the injured elements of the pelvic ring yields better anatomical results. Unfortunately, no clear relationship can be found between quality of reduction/residual displacement and function outcome. Prior to definitive surgical treatment of the unstable pelvic fracture, prophylactic vena cava filter insertion is not recommended. However, early administration of low-molecular-weight heparin has demonstrated a clear reduction in deep vein thrombosis and pulmonary embolism.
