Abstract
Proximal tibia fractures are diverse and represent a wide spectrum of injuries. When evaluating indications for surgery, increasing articular incongruity, joint instability, and joint malalignment are associated with poorer outcomes in non-randomized studies. A meniscectomy should not be performed while early range of motion should be prescribed in proximal tibia fracture treatment. In high energy bicondylar fractures, single midline incisions for double plating should be avoided due to high complication rates. When a bone void requires support from bone graft/substitute, calcium phosphate bone substitute is preferred over iliac crest bone graft or calcium sulphate substitute. There are limited studies directly comparing various treatment options for intra and extra-articular proximal tibia fractures. There is insufficient evidence to recommend specific treatment. The surgeon should use their judgment to select a treatment option which achieves goals of a congruous, aligned, stable knee joint while minimizing complications.
