Abstract
Infliximab (IFX) induces clinical response and remission in ulcerative colitis (UC). However, increasingly a primary goal of therapy is complete mucosal healing. We evaluated the rate and pattern of endoscopic mucosal healing in acute UC patients treated with IFX. 105 acute UC patients with a baseline Mayo score of ≥6, were treated with 3 dose induction IFX and responders provided scheduled maintenance infusions Colonoscopy was performed before the baseline infusion with a follow up evaluation at least 6 weeks after initiating therapy or prior to colectomy. The Mayo endoscopic disease activity score was calculated for each of 6 contiguous anatomic segments (rectum, sigmoid, left colon, transverse, right colon and cecum). A consensus Mayo endoscopic activity score for each segment was recorded by two independent physicians. Endoscopic improvement was defined as a reduction in the follow-up score of at least 1 point from baseline and endoscopic remission as normal mucosa with disappearance of all mucosal lesions.
