Abstract
当院における大腿骨ステム周囲骨折の治療成績を調査した．対象は，2007―2015年に入院加療を行った11例で，入院時平均年齢74.9歳，Vancouver分類は，A：2例，B1：5例，B2：4例であった．Aに保存療法，B1に骨接合術，B2で活動性の低い3例に骨接合術，活動性の高い1例に再置換術を施行し，手術症例は5-8週免荷とした．全例で骨癒合を得たが，B1の1例，B2の骨接合術例全例に術後のステム沈下を，B1の3例，B2骨接合術例の1例に歩行能力の低下を認めた．大腿骨ステム周囲骨折は，B1とB2の判別が困難であるが，B2の骨接合術後の骨癒合は良好で，歩行能力低下率も再置換術と同程度であるため，B2においても骨接合術が治療の選択肢となりうることが示唆された．
