ultrasound IVUS and Optical coherence tomography (OCT) are evolving two newer intra coronary imaging modalities for plaque characterisation.OCT have supremacy over IVUS to understand the plaque morphology in terms of high-resolution (z 10 mm) in recanalised coronaries in Acute coronary syndrome ST segment Elevation Myocardial Infarction (ACS-STEMI) patients.Methods and Results: All patients of STEMI who have angiographically insignificant coronary artery disease on angiography (<50 %) were included in the present study.OCT images were acquired with a commercially available system, using an over the wire OCT catheter (C7 Dragonfly™ and Dragonfly™ OPTIS™, St. Jude Medical).Using the standard validated criteria for plaque characterization OCT images of the lesion were analysed.Variables like lesion length, maximum circumferential width and fibroatheroma cap thickness (thin cap fibroatheroma defined by lipid-rich plaque with cap thickness 65 mm) were assessed by OCT and were found to be more in >40 yr age(group A) as compared to <40 yr age (group B) and the difference was statistically significant (p0.05).About plaque morphology 81.82% of the patient in group A had fibrofatty plaque while in group B 55.56 % of patients had fibrofatty plaque.Fibrous plaque were present in 18.18 % of the patients in group A as compared to patients in group B 44.4% had fibrous plaque.Calcification was found in only one patient (5.56%) in group B while none of the patients had calcification in group A. Residual thrombus were present in 45.45% of patients in groups A. Out of this white thrombus were present in 27.27 % and red thrombus in 18.18 % of patients.In group B 38.88% had thrombus, white thrombi were present in 27.78% and red thrombi were present in 11.11% of the patients.The predominant type of thrombus was platelet-rich white thrombus.Hence there were no significant differences in residual thrombus between two groups suggesting partial recanalization of the vessel with TIMI III flow. Conclusion:The OCT provides more definitive microstructure of coronary plaque characteristic in vivo and helps in assessment and evaluation of plaque in recanalised coronaries and this study provides emerging evidence of unfolding of residual thrombus in recanalised TIMI III flow vessel, thereby optimizing the pharmaco and pharmaco invasive strategy in a given patient for optimal results.
