Abstract
cyclophosphamide, and etoposide followed by 3 cycles of thiotepa/carboplatin, each followed by AHCR.One patient received irradiation following a single transplant that was aborted for toxicity.Two received irradiation at progression.Five patients received no further treatment following AHCR; 1 patient with p53 and a simultaneously diagnosed low-grade glioma received 12 months of temozolomide and celecoxib.Five achieved complete remission (CR) before transplant, 1 being M3.Five remain in CR without irradiation at a mean of 30 months (range, 12 -75 months), including one whose disease has relapsed.Three patients' disease relapsed at a mean of 22 months (range, 12 -42 months); 1 relapse occurred in the spine after 42 months and 2 occurred within the primary site at 12 and 13 months, respectively.CONCLUSIONS: Findings suggest intensive induction chemotherapy followed by myeloablative chemotherapy and AHCR as an initial strategy may permit long-term progression-free survival without irradiation in children with CPC.A larger international meta-analysis is currently underway.
