Editors note: The above letter was referred to the author of the original letter, and Dr. Carrier's reply follows. In reply: Drugs interactions are definitely a most complex process and often of multiple possible origins. Rojas-Femandez and Fisher have written an excellent review of the complexity of pharmacokinetics and of the cytochrome P450 system, in this case illustrated by donepezil. The two patients described in the original report were, as often is the case with Alzheimer's disease, in good health and were not receiving any concomitant medication other than paroxetine and donepezil. We did not have the benefit of having serum levels of donepezil to sustain our hypothesis. As clinicians, we can contribute to the accumulative knowledge by astute observation of individual patients as they are treated in clinical situations. By sharing such observations we hope to stimulate further research.
