Abstract
Harnröhrenstrikturen werden häufig endoskopisch behandelt. Bei Versagen des minimal invasiven Eingriffs sowie bei Patienten mit hohem Rezidivrisiko empfehlen die Leitlinien der American Urological Association eine Urethroplastik. Doch auch bei diesem Vorgehen können Rezidivstrikturen nicht immer verhindert werden. Welche präoperativen Risikofaktoren hierfür prädisponieren, haben kanadische Wissenschaftler von der University of Alberta untersucht. Fazit Die bulbäre Urethroplastik, so das Fazit der Autoren, löst unabhängig von der Operationsmethode in der Mehrzahl der Fälle langfristig die Strikturproblematik. Dennoch werden bei etwa 1 von 14 Patienten weitere Eingriffe erforderlich. Patienten mit Übergewicht, mehreren Komorbiditäten, längeren oder infektionsbedingten Strikturen sollten über das erhöhte Rezidivrisiko aufgeklärt werden. Eine engmaschigere postoperative Nachsorge ist nach Ansicht der Autoren in diesen Fällen sinnvoll.
