В данной статье представлены результаты изучения социально-демографических и клинико-динамических характеристик 152 пациентов с шизофренией с постпсихотической депрессией в контексте феномена душевной боли. Дизайн исследования: открытое обсервационное поперечное исследование в контрольных группах с направленным формированием групп сравнения. Инструменты исследования: диагностика клинических симптомов шизофрении проводилась с применением шкалы PANSS, для детализации депрессии была использована шкала депрессии при шизофрении Калгари. Критериями включения пациентов в общую исследовательскую выборку являлись: верифицированный диагноз «шизофрения» (по МКБ-10), возраст 18–50 лет, отсутствие у пациента выраженных острых психотических симптомов (по шкале PANSS сумма позитивных симптомов <30 баллов), отсутствие выраженных проявлений негативной симптоматики (по шкале PANSS сумма негативных симптомов <35 баллов), письменное информированное согласие на участие в исследовании. Критериями исключения пациентов из общей выборки являлись: наличие у пациента острых психотических симптомов (по шкале PANSS сумма позитивных симптомов >30 баллов), выраженные проявления негативной симптоматики (по шкале PANSS сумма негативных симптомов >35 баллов), наличие экстрапирамидных расстройств. Пациенты были разделены в два этапа на три исследуемые группы: группу А, 103 пациента (постпсихотическая депрессия, значения по шкале депресии PANSS ≥5 баллов), и группу Б, 49 пациентов (отсутствие постпсихотической депрессии, значения по шкале депрессии PANSS <5 баллов). На втором этапе группа А была разделена на две подгруппы: А1, 69 пациентов (постпсихотическая депрессия с душевной болью), и А2, 34 пациента (постпсихотическая депрессия без душевной боли). Критической точкой для разделения на подгруппы явилось значение ≥24 балла по шкале душевной боли. По результатам исследования были сделаны следующие выводы: пациенты с шизофренией с депрессией и душевной болью образуют особую группу, ограниченную социально-демографическими и клинико-динамическими характеристиками – молодые люди преимущественно женского пола, не утратившие работоспособности и социального статуса с относительно ранним дебютом болезни и непродолжительным течением. Наличие душевной боли в структуре аффективных переживаний пациентов не связано с проводимой им лекарственной терапией. This article presents the results of studying the socio-demographic and clinical-dynamic characteristics of 152 schizophrenic patients with postpsychotic depression in the context of the phenomenon of mental pain. Study design: open, observational, cross-sectional study in control groups with a directed formation of comparison groups. Research instruments: the diagnosis of clinical symptoms of schizophrenia was carried out using the PANSS scale; the Calgary Schizophrenia Depression Scale was used to detail depression. The criteria for the inclusion of patients in the general research sample were: verified diagnosis of schizophrenia (according to ICD-10), age 18–50 years, the patient had no pronounced acute psychotic symptoms (according to the PANSS scale, the sum of positive symptoms<30 points), the absence of pronounced manifestations of the defect personality (on the PANSS scale, the sum of negative symptoms <35 points), written informed consent to participate in the study. The criteria for excluding patients from the general sample were: the patient had acute psychotic symptoms (on the PANSS scale, the sum of positive symptoms >30 points), pronounced manifestations of a personality defect (on the PANSS scale, the sum of negative symptoms >35 points), and the presence of extrapyramidal disorders. Patients were divided in two stages into three study groups: group A, 103 patients (postpsychotic depression, PANSS depression scores ≥5 points) and group B, 49 patients (no postpsychotic depression, PANSS depression scores <5 points). At the second stage, group A was divided into two subgroups: A1, 69 patients (postpsychotic depression with mental pain) and A2, 34 patients (postpsychotic depression without mental pain). The critical point for subgrouping was ≥24 on the mental pain scale. According to the results of the study, the following conclusions were drawn: patients with schizophrenia with depression and mental pain form a special group limited by socio-demographic and clinical-dynamic characteristics – young people, predominantly female, who have not lost their working capacity and social status with a relatively early onset of the disease and a short course. The presence of mental pain in the structure of the affective experiences of patients is not associated with the drug therapy being carried out by them.
