Antecedentes: El blastoma pulmonar es una neoplasia maligna, infrecuente y agresiva que representa aproximadamente del 0.25 al 0.5% de las neoplasias malignas primarias de pulmn con una prevalencia menor de 0.25 por milln. Caso clnico: Hombre de 47 aos, tabaquista con disnea progresiva a quien se realizaron estudios de biopsia e inmunohistoqumica y se diagnostic: blastoma pulmonar bifsico clsico. Se estableci tratamiento, sin embargo, el paciente falleci. Conclusin: Este tumor represent un reto, dado que en la literatura no existe un tratamiento con evidencia epidemiolgicamente slida. Relevancia: Esta neoplasia constituye un desafo y este es el primer caso reportado en Colombia.
