Abstract
Red cell transfusion is an extremely common medical intervention. This chapter reviews the current knowledge about red cell transfusion, their risks and benefits, risk posed by anemia, at what hemoglobin concentration a transfusion will mitigate the effects of anemia and result in improved outcomes. It also shows clinical recommendations are largely based on randomized clinical trials identified and synthesized in systematic reviews. Several hemodynamic alterations occur after the development of anemia. The degree of anemia, the specific tissue bed, and a variety of disease processes affect microcirculatory blood flow and oxygen supply. Pathologic processes affecting the microcirculation, particularly prevalent among critically ill patients, may also affect the patient's response to anemia and transfusions. Transfusion thresholds could have differential effects in alternative clinical settings. There are several clinical settings in which oncology patients are frequently transfused with red cells.
