The influence of age and gender on the characteristics of cognitive impairment in patients with cardiovascular risks

Abstract
ЦЕЛЬ ИССЛЕДОВАНИЯ Оценка особенностей нарушений когнитивных функций у пациентов с факторами сердечно-сосудистого риска (ФССР) в зависимости от возраста и пола. МАТЕРИАЛ И МЕТОДЫ В обсервационное поперечное исследование были включены данные 1651 пациента в возрасте от 45 до 84 лет с ФССР из разных регионов Российской Федерации. Наличие и характер когнитивных нарушений (КН) выявлялись с помощью Монреальской шкалы оценки когнитивных функций (MoCA) с учетом общего балла и оценок нарушений отдельных когнитивных функций (память, внимание, речь, исполнительные и зрительно-конструктивные функции, ориентация). РЕЗУЛЬТАТЫ КН выявлены у 89,2% обследованных (общий балл MoCA-теста менее 26). У пациентов среднего возраста (45—49 лет) в большей мере страдали исполнительные функции и внимание. Отрицательная динамика КН значимо ухудшалась в возрастном диапазоне 70—75 лет, статистически достоверно нарастала каждые последующие 5 лет и проявлялась ухудшением как в мнестической сфере, так и по другим когнитивным доменам. Статистически значимые различия показателей общего балла MoCA у женщин и мужчин выявлены в возрасте 80—84 лет (16,1 балла против 13,8 балла соответственно, p=0,0279). К 80—84 годам женщины имели более высокие показатели внимания (p=0,025) и ориентации (p=0,028), чем мужчины этого же возраста. ЗАКЛЮЧЕНИЕ Полученные данные позволяют идентифицировать пациентов независимо от гендерной принадлежности в возрасте до 70 лет как важную целевую группу для диагностики и более агрессивной профилактики КН.
