The role of premorbid personality in the genesis of psychopathological symptomatology in patients with organic anxiety-depressive disorder, schizophrenia and endogenous depression

Abstract
ЦЕЛЬ ИССЛЕДОВАНИЯ Изучение связей между личностными характеристиками преморбидного периода и психопатологической структурой симптоматики у больных органическим тревожно-депрессивным расстройством (ОТДР), шизофренией и эндогенной депрессией (ЭД). МАТЕРИАЛ И МЕТОДЫ Изучен 191 больной (57 — с ОТДР, 93 — шизофренией и 41 — ЭД). Преморбидный период оценивался с помощью Мюнхенского личностного теста и Торонтской шкалы алекситимии; психопатологическая симптоматика — с помощью шкалы SCL-90. Для изучения зависимости психопатологической симптоматики от преморбидных личностных конструктов применялся метод множественной пошаговой регрессии. РЕЗУЛЬТАТЫ Выраженность ведущей психопатологической симптоматики была наибольшей в группах ЭД и шизофрении, наименьшей — в группе ОТДР (p<0,047). Сравнение личностных конструктов преморбидного периода показало наибольшую выраженность невротизма и мотивации при ЭД (64,54±18,56 балла, p=0,005); ригидности, экстраверсии и эзотерических тенденций при шизофрении (52,43±15,78, 44,25±17,08 и 54,97±29,43 баллов соответственно, p<0,007); переносимости фрустрации, тенденций к изоляции при ОТДР (46,86±21,91, 40,75±17,11 баллов соответственно, p<0,02). Регрессионный анализ раскрыл положительную зависимость психопатологической симптоматики депрессии и тревоги при ЭД и шизофрении от выраженности алекситимии (p<0,002) и отрицательную связь переносимости фрустрации с выраженностью тревоги и депрессии при ЭД и шизофрении (p<0,016), а также с величиной тревоги по шкале Zung при ОТДР (p=0,003). Конструкт «Ригидность» не имел статистически значимых связей с тревогой, и депрессией ни в одной диагностической группе. ЗАКЛЮЧЕНИЕ Проведенный анализ выявил преобладание определенных личностных конструктов при каждом из заболеваний, с универсальным отрицательным влиянием алекситимии и положительным влиянием толерантности к фрустрации при тревожно-депрессивных расстройствах любой этиологии.
