Abstract

This Chapter presents a comparative analysis of medical politics and health reforms within three empirical cases in Canada and England. The aim is to understand the medical doctors–healthcare reform nexus and to test the theoretical model’s ability to explain key variations and points of convergence across the cases. The chapter first examines the impact of bargains and policy parameters set at the inception of PFHS on healthcare reforms. Second, it describes how governments address core policy dilemmas in the context of PFHS, such as escalating costs and problems with access to care. Third, it characterises the institution of medical politics as a core feature of contemporary PFHS that acts as a determining influence on healthcare reforms. Fourth, it focuses on the interface of medical doctors and governments’ agentic manifestations to investigate their influence on the evolution of their respective strategies. Finally, the chapter assesses how configurations of the four dimensions of the medical doctor–healthcare reform’s nexus have influenced the content of reformative templates over time. To various extent, the comparative analysis reveals that the interdependent relationship between the medical profession and the PFHS limits prospects for reform.
