Abstract
L’évolution de toutes les disciplines médicales et chirurgicales se fait vers l’hyper- spécialisation, en lien avec le développement de la recherche et des technologies. La population souhaite, à juste titre, pouvoir bénéficier d’une médecine de proximité non spécialisée, et d’une médecine hyperspécialisée, cette dernière étant présente dans les CHU et les établissements de grande taille. Cela doit s’intégrer dans un maillage territorial permettant une gradation des soins intégrant les CHU, les établissements de taille plus modeste et la médecine de ville. L’appareil de formation doit s’adapter en formant des médecins hyperspécialisés et plus généralistes, y compris dans les spécialités médicales. De nouvelles disciplines apparaissent, y compris dans les CHU, telle que la médecine polyvalente hospitalière qui est un exercice médical transversal particulièrement adapté aux filières de post-urgence. Cette organisation de la médecine hyperspécialisée doit s’intégrer dans les évolutions de nos établissements de santé, à travers des plateaux techniques partagés.
