В данной статье представлены результаты изучения клинических характеристик 152 пациентов с шизофренией с постпсихотической депрессией в контексте феномена душевной боли. Дизайн исследования: открытое обсервационное поперечное исследование с направленным формированием групп сравнения. Инструменты исследования: диагностика клинических симптомов шизофрении проводилась с применением шкалы PANSS, для детализации депрессии была использована шкала депрессии при шизофрении Калгари. Критериями включения пациентов в общую исследовательскую выборку являлись: верифицированный диагноз «шизофрения» (по МКБ-10), возраст 18–50 лет, отсутствие у пациента выраженных острых психотических симптомов (по шкале PANSS сумма позитивных симптомов <30 баллов), отсутствие выраженных негативных симптомов (по шкале PANSS сумма негативных симптомов <35 баллов), письменное информированное согласие на участие в исследовании. Критерием исключения пациентов из общей выборки являлось наличие экстрапирамидных расстройств. Пациенты были разделены в два этапа на три исследуемые группы: группу А – 103 пациента (постпсихотическая депрессия, значения по шкале депрессии PANSS ≥5 баллов), группу Б – 49 пациентов (отсутствие постпсихотической депрессии, значения по шкале депрессии PANSS <5 баллов); на втором этапе группа А была разделена на две подгруппы – А1, 69 пациентов (постпсихотическая депрессия с душевной болью), и А2 – 34 пациента (постпсихотическая депрессия без душевной боли). Критической точкой для разделения на подгруппы явилось значение ≥24 балла по шкале душевной боли. По результатам исследования были сделаны следующие выводы: пациенты с шизофренией с депрессией и душевной болью образуют особую группу с клиническими особенностями в каждом психопатологическом домене – позитивном, негативном и общем по шкале PANSS. Для них характерны слабая выраженность апатии, концептуальной дезорганизации, нарушений абстрактного мышления, нарушений мышления в форме стереотипий, а также высокие уровни тревоги, соматической озабоченности и напряженности. This article presents the results of studying the socio-demographic and clinical-dynamic characteristics of 152 schizophrenic patients with postpsychotic depression in context of mental pain phenomenon. Study design: open, observational, cross-sectional study with directional formation of comparison groups. Research instruments: clinical symptoms of schizophrenia were diagnosed using the PANSS scale, and the Calgary Depression Scale for Schizophrenia was used to detail depression. The criteria for patients inclusion in the general research sample were: verified diagnosis of schizophrenia (according to ICD-10); age of 18–50 years; absence of significant acute psychotic symptoms (according to the PANSS scale, the sum of positive symptoms <30 points), absence of significant negative symptoms (on the PANSS scale, the sum of negative symptoms <35 points), written informed consent for participation in the study. The criteria for patients exclusion from the general sample were: presence of significant acute psychotic symptoms (according to the PANSS scale, the sum of positive symptoms >30 points), significant manifestations of personality defect (according to the PANSS scale, the sum of negative symptoms >35 points), and the presence of extrapyramidal disorders. Patients were divided in two stages into three study groups: group A, 103 patients (postpsychotic depression, PANSS depression scores ≥5 points) and group B, 49 patients (no postpsychotic depression, PANSS depression scores <5 points). At the second stage, group A was divided into two subgroups: A1, 69 patients (postpsychotic depression with mental pain) and A2, 34 patients (postpsychotic depression without mental pain). The critical point for subgrouping was ≥24 points on the mental pain scale. According to the results of the study, the following conclusions were drawn: schizophrenic patients with depression and mental pain form a special group with distinct clinical features in each psychopathological domain: positive, negative, and overall on the PANSS scale. They are characterized by a weak severity of violations of the mental sphere: weakly expressed apathy, conceptual disorganization, abstract thinking disorders and thinking disorders in the form of stereotypes, as well as high levels of anxiety, somatic concern and tension.
