Abstract

The antiarrhythmic agent amiodarone has been associated with hyperthyroidism in up to 10% of patients. We observed a 53-year-old man who experienced thyrotoxicosis 28 months after initiation of amiodarone therapy. Despite a dispensing error that caused him to be supplemented with triiodothyronine, he remained asymptomatic, with no tachycardia, tremor, or thyroid inflammation, while continuing to receive amiodarone. Only when his free-T4 concentration reached 92.9 pmol/L (upper limit of normal: 25 pmol/L) did he develop atrial fibrillation and tremor.
