An 85-year-old man was admitted with circumferential pericardial and bilateral pleural effusions, with a rapidly evolving mass noted on follow-up echocardiography. Imaging workup demonstrated characteristic findings of primary cardiac lymphoma, such as a floating vessel sign. This allowed for early biopsy and chemotherapy initiation. The novel teaching points of this case report address an approach to early diagnosis based on radiographic findings and mitigation of certain barriers to tissue diagnosis utilizing an uncommon percutaneous biopsy technique when features preclude standard biopsy techniques.
