Abstract
Seksualność jest istotnym aspektem życia, także dla pacjentów zbliżających się ku jego końcowi.Jednak funkcje seksualne w tej grupie pacjentów ulegają pogorszeniu w związku z chorobą podstawową, jej objawami, chorobami towarzyszącymi lub stosowanym leczeniem.Istotnym czynnikiem mogą być szeroko stosowane w opiece paliatywnej opioidy, prawdopodobnie wpływające na funkcje seksualne głównie poprzez hamowanie osi podwzgórze-przysadka-gonady.Skutkuje to zaburzeniami erekcji, pobudzenia seksualnego, pożądania oraz zmniejszoną ogólną satysfakcją z życia seksualnego.Ze względu na skąpe dane ciężko jest jednoznacznie wskazać opioidy charakteryzujące się niższym ryzykiem, wydaje się, że mogą to być buprenorfina, tapentadol, tramadol oraz opioidy krótkodziałające.Opcje terapeutyczne są nieliczne, a dowody pochodzą z pojedynczych badań i obejmują: terapię zastępczą testosteronem, bupropion, trazodon, antagonisty opioidowe, a także żeń-szeń.Częsta wielochorobowość pacjentów objętych opieką paliatywną wymaga brania pod uwagę również innych czynników etiologicznych oraz nakierowane na nie metody leczenia, utrudnia też wnioskowanie na temat istotności wpływu opioidów na funkcje seksualne.
