La téléconsultation en psychogériatrie : facteurs de succès et nouvelles pratiques

Dans le domaine de la télémédecine, la psychiatrie bénéficie d’une avance de pratiques. Elle a été une spécialité pionnière dans la mise en œuvre du suivi à distance (Norman, 2006). Dans cette communication, nous traitons du cas de la téléconsultation en psychogériatrie entre hôpital et EHPAD (Établissement d’hébergement pour personnes âgées dépendantes) et analysons les enjeux socio-organisationnels desquels découlent le succès et l’adoption de ces nouveaux dispositifs. Nous avons souhaité comprendre quels pouvaient être les facteurs facilitant ou au contraire freinant l’adoption du dispositif utilisé en menant une étude qualitative. Nous avons réalisé une série de 12 entretiens à différentes étapes du projet entre 2018 et 2019 avec les principaux porteurs et utilisateurs du projet de téléconsultation. Une observation d’une téléconsultation sur place a également pu être réalisée pour comprendre le déroulé de la téléconsultation. Les résultats apportent un éclairage sur les aspects sociotechniques influençant l’appropriation des dispositifs de téléconsultation en psychogériatrie. La recherche a aussi permis de décrypter comment le dispositif permet de matérialiser de nouvelles formes de collaboration entre les professionnels de santé, mettant la pluridisciplinarité au cœur de l’organisation. Elle a également mis en lumière de nouveaux repères spatio-temporels et modaux, en révélant des formes d’organisation proches de celle des équipes mobiles.
