La sfilis congnita es causada por la infeccin del feto con Treponema pallidum durante el embarazo.Los sntomas son variables.Si bien es comn el dao endotelial, no suele estar presente en los casos congnitos.Reportamos el caso de un lactante de 42 das de vida hospitalizado por masa abdominal.Las imgenes confirmaban la presencia de una lesin en el lbulo heptico izquierdo sin efecto de masa.Las biopsias mostraron cambios compatibles con infarto y hepatitis neonatal.Las serologas del paciente y de su madre confirmaron el diagnstico de sfilis congnita, y recibi tratamiento con penicilina intravenosa.El hgado se encuentra protegido de los daos isqumicos gracias a su doble irrigacin, pero la acumulacin de noxas puede haber provocado dicha presentacin inusual.Tres meses ms tarde, el paciente se encontraba libre
