ALTERAÇÕES HISTOPATOLÓGICAS CARDÍACAS CAUSADAS PELA DOENÇA DE CHAGAS

Abstract
anlise de literatura sobre histopatologia da tripanosomase americana, nas bases de dados PubMed, SCIELO e CAPES, utilizando-se os seguintes descritores, em portugus e ingls: doena de Chagas, histopatologia da doena de Chagas e Trypanosoma cruzi. RESULTADOS: A infeco chagsica manifesta duas fases: a fase aguda, associada infestao e multiplicao parasitria nos tecidos, e a fase crnica, com a mitigao da parasitemia e das reaes inflamatrias sistmicas. A hipocontratilidade do miocrdio gera lentificao de fluxo e favorece trombose nas cmaras cardacas. Macroscopicamente, a miocardite crnica chagsica apresenta trombos parietais nos trios e ventrculos e, ao sofrer dilatao global de todas as cmaras, o corao adquire um contorno arredondado, lembrando uma moringa. caracterstico da doena de Chagas o adelgaamento do miocrdio na ponta do VE, formando um aneurisma de ponta acompanhado por fibrose. H hipertrofia do miocrdio ventricular, mas esta torna-se mascarada pela dilatao total. Microscopicamente, a fase aguda da miocardite chagsica caracterizada por intenso parasitismo dos miocardicitos, sendo possvel a observao de ninhos de leishmanias como reduzidos pontos basfilos no interior das fibras do miocrdio. No interstcio h intenso infiltrado inflamatrio de clulas mononucleares e neutrfilos, assim como, a intensa hipertrofia vicariante das fibras do miocrdio. CONCLUSO: Conclui-se, portanto, que a escassez de estudos brasileiros acerca das alteraes histopatolgicas cardacas desta enfermidade mostra-se preocupante, pois a doena de Chagas, apesar do seu declnio nos ltimos anos, ainda compreende um problema de sade pblica. Os estudos e relatos de caso permitem um maior compreendimento do curso clnico e prognstico da doena, culminando em terapias alternativas e eficazes para a maior qualidade de vida dos portadores.
