and Importance: Anatomical variations are widely described in the anterior communicating artery (AComA) complex of the circle of Willis. Some variants may be associated with increased risk of aneurysm formation and rupture and impose a challenge for surgical interventions in treating aneurysm. Clinical Presentation: In current paper, we reported the case of a 74 years old woman who was admitted to hospital with subarachnoid hemorrhage. She was then diagnosed via preoperative computed tomography angiography to have concurrent AComA duplication and aneurysm with triplicate anterior cerebral artery. Patient underwent microsurgical clipping of the aneurysm with straight fenestrated titanium clip with good surgical and post-operative outcome. Preoperative diagnosis of these rare anatomical variations though appropriate imaging study is warranted to take optimal approach in procedure and minimize the risk of complications during or after the intervention.
