Introduction La recanalisation au premier passage (first-pass effect [FPE]) est corrélé avec un bon résultat fonctionnel chez les patients victimes d'accident vasculaire cérébral ischémique. Il est également utilisé comme un marqueur d'efficacité de la thrombectomie mécanique (TM). Dans cette étude, on évalue la performance d'un modèle prédictif de deep learning basé sur les données radiomiques du thrombus en comparaison avec des experts et les données radiologiques non radiomiques du thrombus. Méthodes Les données viennent de l'essai “The Efficacy and Safety of Nerinetide for the Treatment of Acute Ischemic Stroke” (ESCAPE-NA1) qui a comparé le neuroprotecteur nerinetide au placebo chez les patients recevant une TM. Nous avons inclus tous patients avec des examens tomodensitométriques (TDM) et angio-TDM disposant de coupes fines (épaisseur ?2.5 mm). La segmentation manuelle des thrombi était réalisée par des radiologues entrainés. Nous avons divisé les données en 3 échantillons: échantillon de dérivation (entrainement [n=388] et validation [n=55]) et de test (n=111). Le modèle prédictif a été comparé à trois experts en neurovasculaire et aux données non radiomiques du thrombus (longueur, volume, densité et perméabilité du thrombus) pour la prédiction du FPE sur l’échantillon test en utilisant l'aire sous la courbe ROC (AUC-ROC). Résultats Parmi les 554 patients inclus, FPE était observé dans 176 (31.7%) patients. Un modèle prédictif en utilisant les données radiomiques du thrombus sur la TDM a été développé et testé. Ce modèle était le plus performant pour la prédiction de FPE (AUC=0.74 [95% CI: 0.64, 0.84]). Ce modèle était supérieur aux trois experts (AUC= 0.62 [95% CI: 0.53, 0.71], p=0.01) et aux caractéristiques non radiomiques du thrombus (AUC= 0.53 [95% CI: 0.44,0.63], p<0.001). Conclusion Un modèle prédictif basé sur la radiomique du thrombus en TDM performe mieux que les experts et les données non-radiomiques du thrombus pour la prédiction du FPE au cours de TM.
