Abstract
Introducción: Los hallazgos de la colangioscopia digital de un solo operador (DSOC) logran alta precisión en el diagnóstico de lesiones neoplásicas de la vía biliar (N).Sin embargo, los acuerdos intra e interobservador de los endoscopistas varía ampliamente.Objetivo: Validar clínicamente un nuevo modelo de IA comparándolo con endoscopistas expertos.Métodos: Ensayo diagnóstico multicéntrico.Cuatro endoscopistas expertos, ciegos a la clínica, observaron y clasificaron vídeos de lesios biliares N vs. no neoplásicas (NN), basados en los criterios de Robles-Medranda et al y Mendoza.Esto fue comparado con la IA, también ciega a los vídeos.El patrón oro fue el seguimiento clínico a un año.Se estimó un n = 170, con una proporción N vs. NN 50:50.NCT05147389.Resultados: Se analizaron 170 videos de 170 pctes, de 4 centros y 4 países.La IA obtuvo una sensibilidad > 90%, especificidad > 60%, VPP > 70% y VPN > 85%.La precisión de la IA es comparable e incluso podría ser estadísticamente superior a la de un endoscopista experto (tabla 1).Conclusión: En nuestro conocimiento, actualmente este es el estudio con el mayor número de casos de DSOC analizados por IA a nivel mundial, además del único en donde se compara la IA vs. endoscopistas expertos.El modelo de IA propuesto reconoció con precisión entre las N vs. NN al mismo nivel de los expertos e incluso podría superarlos significativamente
