Abstract
Fenestration of the herniated disk space at the time of decompressive surgery is recommended to prevent further extrusion of disk material through the ruptured annulus fibrosus in the early postoperative period. Prophylactic fenestration of unaffected, adjacent thoracolumbar (TL) disks has been shown to significantly reduce confirmed late recurrence in dogs undergoing prophylactic disk fenestration in the TL region. Right-handed surgeons typically find that TL fenestration is easily performed on the left side of the spine, but since fenestration is most commonly performed with concurrent decompression, the approach will depend on the side of the lesion and the preferred surgical approach for decompression. Increased cost associated with longer surgical and anesthetic times, increased surgical incision length, and possibly postoperative morbidity related to additional tissue dissection and surgical trauma should however be considered on an individual basis. Dogs that undergo multiple fenestrations may have a longer incision, which may be more painful in the initial postoperative period.
