Résumé Au tournant des années 1970, un peu partout dans le monde, l’expression « santé communautaire » était très répandue dans l’univers sociosanitaire. Inspirée de sources principalement latino-américaines et étasuniennes, elle était centrée sur une approche multidisciplinaire et participative de la dispensation des services de santé, tant préventifs que curatifs. Qu’en est-il aujourd’hui ? Cet article se propose d’étudier l’évolution sociohistorique de la santé communautaire depuis 40 ans. Pour cela, un modèle qui positionne la santé communautaire et la santé publique comme deux domaines dans le champ sociosanitaire est d’abord présenté. Selon les lieux, les époques et la capacité des différents acteurs concernés à faire valoir leurs intérêts, l’espace couvert par ces deux domaines et leur articulation prennent des morphologies différentes. Ces dynamiques sont illustrées ici par l’analyse de l’espace occupé par la santé communautaire et la santé publique en France et au Québec entre 1970 et 2010. On peut conclure que la santé communautaire est toujours un « faire » au sens de Fassin, une pratique qui se distingue par sa référence à la participation et son insertion dans une démarche de développement social. Aujourd’hui, davantage que comme un espace différencié (« la » santé communautaire avec ses structures propres), c’est « l’approche communautaire » de la santé qui émerge comme l’une des modalités d’intervention des divers acteurs publics.
