Abstract
Contexte : Le técovirimat (TCV, TPOXX MD ) est un médicament antiviral indiqué pour le traitement de la variole.Il existe également une base théorique pour son utilisation dans des cas de mpox.Toutefois, son approbation a été fondée sur des études animales, et son efficacité et son profil des effets secondaires chez les patients humains atteints de la maladie sont inconnus.Méthodes : Au cours de l'épidémie internationale de mpox de 2022, les cliniciens canadiens ont eu accès au TCV de la Réserve stratégique nationale d'urgence de l'Agence de la santé publique du Canada pour les cas graves de mpox.Nous décrivons l'utilisation du TCV chez neuf adultes atteints d'une infection sévère par le virus de la mpox à Montréal, au Canada.Résultats : Cinq patients ont été traités pour des symptômes graves et potentiellement mortels de la sphère oropharyngée, tandis que quatre ont été traités pour des atteintes génito-urinaires ou ano-rectales.Deux tiers des patients ont également été traités pour une suspicion de surinfection bactérienne.Tous les patients se sont rétablis (délai médian de résolution des symptômes graves : neuf jours) sans rechute ni réadmission à l'hôpital.Aucun patient n'a signalé d'effets indésirables attribuables au TCV et aucun patient n'a interrompu son traitement.Conclusion : Notre expérience suggère que le TCV est bien toléré et pourrait accélérer la guérison dans les cas graves.Ces données observationnelles préliminaires pourraient également être expliquées par le traitement concomitant des surinfections bactériennes et sont limitées par l'absence d'un groupe témoin.Des essais cliniques contrôlés doivent être menés pour clarifier le bénéfice attribuable au TCV dans les cas d'infection grave par le virus de la mpox.
