Abstract
L’attivazione a lungo termine del sistema nervoso simpatico gioca un ruolo importante nella progressione dello scompenso cardiaco e nel rischio di morte improvvisa. Il ruolo del beta-bloccanti nei pazienti con insufficienza cardiaca rappresenta la pietra miliare della terapia dello scompenso cardiaco attraverso i risultati di molteplici studi. I beta-bloccanti migliorano significativamente la funzione ventricolare, la morbilità e la mortalità. Pertanto, questi farmaci devono essere somministrati con cautela e successivamente titolata la dose poiché all’inizio si assiste ad un peggioramento transitorio dei sintomi. Una volta iniziato il trattamento e raggiunto il dosaggio di mantenimento, il trattamento deve essere continuato al dosaggio massimo tollerato dal paziente. In questa review si discuterà del ruolo dei BB, dai risultati dei primi trial alla pratica clinica quotidiana.
