Orbitozygomatic Approach for a Ruptured Basilar APEX-P1 Perforator Aneurysm

Abstract
A 64-year-old male presented Hunt-Hess:4 and modified-Fisher:4. CTA/DSA depicted right-sided aneurysm either of SCA/P1 origin. An orbitozygomatic craniotomy and transcavernous approach was performed. Dissection and expansion of the corridor through the oculomotor-triangle, allowed exposure of ipsilateral SCA, bilateral-P1, basilar-P1 perforators, and aneurysm dome. Temporary clips on ipsilateral P1 and PComm were positioned, and permanent clip was placed on the perforator near Basilar-P1 junction. ICG video angiography confirmed patency of unaffected perforators without aneurysm filling. Follow-up angiogram showed total obliteration of the aneurysm.
