FISIOPATOLOGIA DA DOENÇA DE ALZHEIMER E A FARMACOLOGIA DOS SEUS TRATAMENTOS MODIFICADORES

Abstract
Doença de Alzheimer (DA) é a principal patologia responsável pelo depauperamento cognitivo em adultos, com prevalência em idosos, respondendo por isso pela metade dos diagnósticos de demência. Conforme constatado nas pesquisas já publicadas, a idade é o fator determinante de risco, tendo prevalecimento menor que 1% na faixa etária de 60 a 64 anos, e alcançando 2/5 da população do grupo etário entre 90 e 95 anos. O problema de pesquisa que fundamenta o presente trabalho é o conhecimento ainda incipiente sobre a fisiopatologia e as limitadas opções de tratamento farmacológico da modificação da Doença de Alzheimer consubstancia. Para tanto se tomou como hipótese explicativa para a etiologia da DA à cascata amiloidal e a hipótese colinérgica. O objetivo do texto é discutir os achados fisiopatológicos e farmacológicos das pesquisas em saúde sobre os tratamentos modificadores da Doença de Alzheimer, e especificamente identificar bibliograficamente os aspectos fisiopatológicos da DA pela perspectiva das hipóteses da cascata amiloidal e colinérgica, e descrever os tratamentos farmacológicos com Inibidores da acetilcolinesterase (I-ChE) e com memantina na neurotransmissão glutamatérgica. Metodologicamente o trabalho constitui-se de pesquisa básica explicativa, de abordagem qualitativa e dedutiva, tendo como procedimento técnica a pesquisa bibliográfica. Constatou-se que os tratamentos farmacológicos com Inibidores das acetilcolinesterase (I-ChE) e com memantina na neurotransmissão glutamatérgica, se dá com o uso da tacrina, rivastigmina, donepezil e galantamina, tendo o trabalho nesse sentido, apresentado a descrição técnica e científica dos I-ChE, bem como a posologia e os efeitos adversos desses tratamentos.
