A síndrome da fragilidade é um importante indicador da condição de saúde em idosos. A escala de fragilidade de Edmonton é um instrumento multidimensional, que avalia a síndrome da fragilidade, validado e adaptado para o Brasil. O presente estudo é epidemiológico, observacional e transversal. Teve como objetivo, investigar a ocorrência de fragilidade nos idosos acompanhados na atenção primária à saúde. Através da escala de fragilidade de Edmonton, foi avaliada a ocorrência da síndrome de fragilidade em 109 idosos com idade média de 71,5 ± 5,5 anos. Observou-se que 22,4% foram classificados como vulneráveis e 10,3% apresentavam algum grau de fragilidade. Com relação aos domínios da escala de fragilidade, os que apresentaram maior ocorrência foram: baixo desempenho físico, percepção da saúde razoável/ruim, sentir-se triste ou deprimido, dificuldade de controlar o esfíncter e dependência funcional em duas ou mais atividades da vida diária. Conclui-se que a escala de fragilidade de Edmonton se constitui como um importante instrumento na investigação da síndrome da fragilidade geriátrica no âmbito da APS, à medida que seus domínios podem ser elencados e identificados pelos profissionais no planejamento e realização do processo de cuidado dos idosos acompanhados.
