Abstract
Planowanie opieki z wyprzedzeniem (advance care planning -ACP) to proces komunikacji między pacjentem, jego bliskimi a personelem medycznym i innymi osobami zaangażowanymi w opiekę polegający na uzgodnieniu planu przyszłej opieki z uwzględnieniem preferencji chorego.Zaplanowanie i ustalenie celów przyszłej opieki jest wyrazem autonomii decyzyjnej chorych i stanowi podstawę do świadczenia holistycznej i zindywidualizowanej opieki.Praktyki ACP od czasu wprowadzenia ewoluują.Obecnie podkreśla się znaczenie rodziny, empatycznej komunikacji i interwencji skierowanych bezpośrednio do pacjenta.Planowanie opieki z wyprzedzeniem nie jest w Polsce rozpowszechnione, brak jest również odpowiednich regulacji prawnych w tym zakresie.
