Abstract
Investigating for fatigue in transportation occurrences using a systems approach can identify factors that contribute to fatigue in transportation operations, document when fatigue does (and does not) contribute to an occurrence, and communicate the findings publicly to improve safety in the future. It is important that investigation reports tell the complete story so that appropriate mitigations can be identified and implemented. This chapter reviews the Transportation Safety Board of Canada (TSB)’s fatigue investigation methodology, including some of the associated challenges. The importance of the investigative process in identifying where organisational and/or shift scheduling practices contribute to fatigue risk is illustrated by some recent TSB investigation “stories” where fatigue was found to play a role.
