La réponse au nombre croissant de patients qui consultent pour des problématiques psychothérapeutiques sévères et pour qui la cure-type n’est pas indiquée aura pris la forme d’aménagements du dispositif-cadre de soin. Prendre en considération le cadre inscrit dans la transitionnalité proposé à ces patients permettra de réfléchir tant aux impacts que ces remaniements ne sont pas sans entrainer qu’au potentiel thérapeutique que ceux-ci présentent. On verra comment des approches psychanalytiques qui ne sont pas entièrement vectorisées par le langage, notamment la psychothérapie à médiation artistique, sont susceptibles, grâce à leur pouvoir d’objectivation et leur capacité à rendre figurable , de « faire apparaître [le] conflit au dehors, de manière à en permettre une introjection secondaire » (Baldacci, 2010). Il sera également question de reconnaître ce que le cadre thérapeutique réinventé doit aux patients eux-mêmes (Birot, 2018) et la disposition du psychothérapeute à l’instaurer, ce qui, dans le contexte de la médiation artistique, ne saurait advenir que par la prise en considération de la figurabilité de l’image dans sa double valence, à la fois matérielle et psychique.
