Pacjentom w okresie agonii i preagonii towarzyszą kliniczne objawy odwodnienia lub przewodnienia.Mają one wiele przyczyn fizjologicznych, lecz mogą także być związane z chorobami lub z przyjmowanymi przez pacjentów lekami, lub rodzajem terapii (chemo-i radioterapia).Objawy związane z odwodnieniem dotyczą prawie wszystkich układów i powinny być monitorowane przez personel medyczny, gdyż ich lekceważenie może prowadzić do pogorszenia jakości życia pacjenta oraz wtórnych dolegliwości.Wykazano jednak, że w ostatnich dniach życia pacjent może także odnieść korzyści wynikające z odwodnienia.Parametry laboratoryjne są dyskusyjnym wskaźnikiem odwodnienia i nie należy przypisywać im decydującego znaczenia.W schyłkowym okresie choroby pacjent może również doświadczać przewodnienia, które także manifestuje się klinicznie.Nadmiar wody w ustroju może sprawiać, że pacjent odczuwa dyskomfort, jednak może również odnieść pewne korzyści, np.związane z lepszą dystrybucją leków w organizmie.Nie ma zatem jednomyślności przemawiającej na korzyść lub przeciwko nawadnianiu chorego w schyłkowej fazie choroby.
