PremessaNella pratica infermieristica che interessa i pazienti affetti da disturbi mentali, le criticit caratterizzate da realt/situazioni rimangono nell'ombra e sono indagate solo parzialmente.Ci pu determinare frus-trazione tra gli operatori, una presa in carico non adeguata e soluzioni parziali dei problemi assistenziali di pazienti particolarmente fragili con bisogni latenti.Modalit di handover strutturate tra professionisti afferenti ai vari servizi possono fare la differenza nella gestione della continuit assistenziale e della personalizzazione delle cure.
