Abstract
Amaç: Bu çalışmanın amacı, laboratuvarımızda son 5 yılda gastroenterit ön tanısı almış hastalardan alınan örneklerde viral etken profilini değerlendirmektir.Gereç ve Yöntem: Bu çalışmada, Nisan 2017-Ekim 2022 tarihleri arasında İstanbul Üniversitesi-Cerrahpaşa Tıp Fakültesi Tıbbi Mikrobiyoloji Laboratuvarına gönderilen akut gastroenterite (AGE) neden olabilecek viral patojenlerin saptanması için gelen 354 gaita örneği incelenmiştir.Çalışmada multipleks sendromik gastrointestinal panelde taranmak üzere 5 farklı viral AGE ajan (İnsan Adenovirüsü F40/F41, Norovirüs GI, Norovirüs GII, Rotavirüs A, Astrovirüs ve Sapovirüsler) saptanan örneklerin dağılım yüzdelerine bakılmıştır.Laboratuvarımızda multipleks sendromik gastrointestinal panelde (QIAstat-Dx® Gastrointestinal Panel, Real-Time PCR, Qiagen, Almanya) çalışılan bu dışkı örneklerinden elde edilen sonuçlar retrospektif olarak değerlendirilmiştir.Bulgular: 2017-2022 arasında AGE şüphesiyle gelen 354 gaita örneğinin 105 tanesi viral etken yönünden pozitif bulunmuşken, 249 tanesinde ise viral patojen saptanmamıştır.Pozitif bulunan 105 örnekte; 4 tane Astrovirus, 17 tane Adeonovirus, 69 tane Norovirus, 1 tane Sapovirus ve 14 tane de Rotavirus saptanmıştır.Sonuç: Enterik virüslerin saptanmasında nükleik asit amplifikasyon testlerinin (NAAT) kullanılması, bu virüslerin akut gasrtoenteritlerdeki rolünün ve kapsamının aydınlatılmasında büyük bir ilerleme olmuştur.AGE tablolarında olası viral etkenlerin hızlı ve güvenilir bir tanı yöntemi ile saptanması, özellikle bağışıklığı baskılanmış hasta gruplarında gereksiz antimikrobiyal ilaç kullanımı da önlenmiş olacaktır.Potansiyel AGE ajanlarının hızlı ve doğru tayini; tedavi, hastaneye yatış, enfeksiyon kontrolü ve hastanın sağlığına kavuşması açısından uygun kararlar verilmesine ve kontrol önlemlerinin daha hedefe uygun olarak uygulanmasına yardım edecektir.
