Abstract
Understanding wire technology and the construction of coronary guidewires allows for better usage of the wire and increased technical success rates. This is particularly important given that the predominant mode of procedural failure in chronic total occlusion (CTO) percutaneous coronary intervention (PCI) using the contemporary “hybrid” approach has been the inability to cross the lesion with a guidewire. Hydrophilic, polymer-jacketed wires can more easily enter the subintimal space and cause dissections and perforations. The antegrade wiring strategy involves sequentially moving through guidewires of increasing tip stiffness to puncture the proximal cap of a CTO. A widely used wire for intraplaque navigation is the Gaia series from Asahi Intecc. Tactile feedback is very important in CTO PCI to ascertain the position of the wire within the lesion. The main movements used in guidewire manipulation are rotational and advancement-retraction movements.
