Abstract
Abstract None of the models for the final common pathway of schizophrenia described thus far can approach the explanatory power that the circulatory system offers for heart failure. Yet we have some emerging principles. It seems highly likely that there is some deficit in either GABAergic or NMDA receptor circuitry related to an early neurodevelopmental genetic, viral, or hypoxic anomaly. The deficit is then manifested with cortical pruning at around adolescence, leading to dysregulation of dopamine release, particularly in response to stress. Whether there are further degenerative changes after the onset of illness remains debatable, but there is certainly ample evidence that the loss of gray matter proceeds for several years in many patients.
