Abstract
Each spring at a certain medical school, the first year students are given a one hour lecture on alcoholism as part of their course in psychiatry. Following the lecture, small groups of medical students gather and each group interviews an individual who has a problem related to the subject of the lecture. This chapter discusses some issues which have not yet had their full impact on those primarily involved with intervention, either because the implications of certain ideas would create major problems for the treating institutions or because the ideas would force us to reorder and complicate familiar notions that are easy to understand and communicate or simply because fashion stands in the way. The overwhelming proportion of formal treatment programmes in the United States, Canada and a number of other Western countries are built solidly around affiliations with Alcoholics.
